Request a Consultation
If you are an existing patient, please email us or call us on 02 9709 4700 to request a consultation. You may need a new referral from your GP if your referral has expired (e.g. if the referral used previously is now more than 12-months old).
A member of our team will work with you to schedule in the date and time for your consultation.
If you are a new patient, please use this link to download our New Patient form PDF. Please email your completed New Patient form, and a referral letter from your GP, to enquiries@reproductiveimmunology.com.au.
A member of our team will contact you as soon as possible to arrange your appointment.
After booking, if you wish to cancel or reschedule, it is greatly appreciated if you can give at least a week’s notice. This will allow us to offer the appointment slot to another patient. You can learn more about our cancellation policy on our FAQ page.
